sent to the Music Festival Chairman. Required fields are
noted with *** to the right of the entry box.
NOTE: The purpose of the Performer
Registration Form is to identify the performers (either
quartet or chorus) and the events they wish to participate
in. Filling out this form is only done by the performing
group contact person.
However, each performer must still register for the Music
Festival by going to the Ticket Order Form and entering the
appropriate information.
